1. Submit your completed application form to the centre reception, allow up to 28 days for processing.
2. If your current Active Concession card has been in existence for more than 3 months, continuing proof of eligibility is required.
3. Upgrades and changes to Leisure Active categories are at discretion of Fife Sports and Leisure Trust. A minimum of 30 days notice is required using the amendment form.
4. Fife Sports and Leisure Trust reserves the right to reject an application.
5. Lost/stolen Leisure Active cards can be reported at any leisure centre. This should be done as soon as possible. A charge of £2.50 will be made for a replacement card.
6. The Leisure Active card is not transferable.
7. Leisure Active Payment methods:
Full Payment:
Card valid from day of purchase for 12 months.
Direct Debit:
(I) First payment should be made at the leisure centre reception on completion of the application form. New members only.
(ii) Membership is valid from date of first payment.
(iii) For Junior applicant's parent or guardian must complete the Direct Debit mandate.
(iv) Any member paying by Direct Debit will be notified, in writing, of any annual changes to their Direct Debit payment. The scheme will continue to offer unlimited access without disruption or inconvenience. To opt out at any time, after the minimum contract period, 30 days written notice is required using the cancellation form.
(v) If a direct debit payment is not received, a letter will be sent to the member advising of the amount due and that the membership has been suspended. The member will be advised that they will not be permitted to rejoin the scheme until all amounts due are paid in full. If the outstanding amount remains unpaid, the Leisure Active membership will be cancelled.
8. Refunds for Leisure Active memberships paid in advance will only be made in exceptional circumstances on a pro-rata basis. The 30 Day Pass is non-refundable.
9. The Leisure Active charges will be reviewed annually and determined by Fife Sports and Leisure Trust.
10. The Leisure Active member must present their card on every visit and notify of every activity that they wish to attend.
11. Failure to attend a pre-booked activity may result in the suspension and or cancellation of your Leisure Active membership.
12. Bookings can only be accepted if a valid Leisure Active card is produced, or in the case of a telephone booking the membership number is quoted.
13. Fife Sports and Leisure Trust reserves the right to exclude Leisure Active members if their behaviour is deemed inappropriate or constitutes a risk.
14. Junior members aged 5 and up to and including 13 years are only eligible for Bronze membership (swimming only).
15. Junior members aged 5, 6 & 7 years must be accompanied by an adult in the swimming pool.
16. Only authorised users will be admitted to the gym. Proof of competence must be produced.
17. Access will be restricted during seasonal holidays and maintenance closures. There will be no refund for any variations in access times or for the restriction to facilities.
18. To access the health suite all members must be 16 years and over.
19. Attempted use of Leisure Active card by a non member will result in cancellation of that Leisure Active membership.
20. All facilities and activities are subject to availability and are governed by the Sports and Leisure Venue Rules.
21. All category options subject to availability, customers should familiarise themselves with what is available in each centre.
22. Data collected in accordance with the Data Protection Act 1998.
23. Fife Sports and Leisure Trust reserves the right to amend terms and conditions at any time.
24. Membership is for a minimum period of 4 months. Members can cancel at any time after 3 months of membership has been completed but must give 30 days notice in writing using the cancellation form, otherwise they will be invoiced for this final month. The cancellation form should be posted to Leisure Active Administration Team, Duloch Leisure Centre, Nightingale Place, Dunfermline KY11 8LW.
